Clean up frmIndex.vb: remove legacy and unused code

Removed large blocks of commented-out legacy code, redundant imports, and obsolete alternatives throughout frmIndex.vb. Simplified logic for Multi-Indexing UI, file handling, and folder creation by retaining only active code. These changes improve readability and maintainability without altering functionality.
