Add "Notes" section to document pending toolbar polish

The "Approval Gates" section in `SENDER_PDF_TOOLBAR_PLAN_EN.md`
was updated to include a new "Notes" subheading. A checklist
item was added under this section to indicate that visual
polish for the toolbar (e.g., icon sizing, balance, and
spacing adjustments) is still pending. This change provides
clarity on outstanding design tasks.
